Missing Receipt Affidavit 

The Missing Receipt Affidavit is used when an original itemized receipt cannot be obtained after all reasonable efforts have been made to secure a replacement from the merchant. This form should be completed by the cardholder and submitted with the applicable expense documentation. The affidavit serves as supporting documentation only and should be used sparingly. Frequent use of this form may result in additional review or require further justification.

T&E Maintenance Form 

This form is used to request changes to a cardholder's account, including updates to account information, departmental transfers, credit limit changes, and other account maintenance requests. It is also used to report a lost or stolen card. Please complete all applicable sections of the form and submit it to the Corporate Card Administrator for processing.
